Transitioning, Get on Board!
Participants will gain an understanding of transition planning and how a coordinated set of activities in a plan will lead to a smooth transition from school to the adult world.

 

Entitlement to Eligibility
While in school, individuals are entitled or expected to receive services. In the adult service world, individuals receive services based upon their eligibility. This training identifies the differences.

Office of Disability Employment Policy (ODEP) - Youth
The Office of Disability Employment Policy offers The Guideposts for Success -- key educational and career development interventions that can make a positive difference in the lives of all youth, including youth with disabilities.  It promotes experiences, transition activities, family involvement, and leadership development.

What is MTP?
MTP stands for Model Transition Program. It is a grant from VESID to improve the transition process for students with disabilities preparing for life after High School. There are 60 MTP projects, 150 high schools, and 7 regions across New York State. Learn more about MTP
