Improving the transition process for students with disabilities to reach their potential in adult life

About Us

 

The New York State Model Transition Program is an initiative between the New York State Education Department (NYSED), Cornell University, and the University at Buffalo (UB). The Model Transition Program (MTP) at UB is designed to establish model collaborative service delivery partnerships to improve the transition process for students with disabilities preparing for life beyond high school.

 

NYSED's Office of Vocational and Educational Services for Individuals with Disabilities funds the Model Transition Program and manages the contracts and activities of each of the 60 funded projects throughout the New York State. Cornell University's Employment and Disability Institute provides evaluative research and disseminates findings. UB's MTP provides individualized training and technical assistance support for 60-school-agency partnerships (emcompassing over 150 school districts) throughout the state.

 

UB's MTP is one of the projects operating under the Center on Rehabilitation Synergy, housed within the UB's Graduate School of Education.
